You will encounter Nerius's Bloodfinger as soon as you enter Murkwater Cave in Limgrave. He is an NPC Invader in Elden Ring and will immediately start attacking you. Defeating him will be almost impossible as you will only have access to basic armor and weapons at the start. Fortunately, Yura the Blood Finger Hunter will help you in this battle.After entering the cave, drag Nerius to the bridge nearby. On the way you will meet a giant dragonfly. Quickly eliminate him and continue towards the bridge. Stop at least a few hundred meters before the bridge. There will be several skeletons below him, so keep enough distance. If they start attacking you too, then all hell will break loose and you will inevitably die.
